This form is designed to collect the key information needed to process your application for MCS certification, to enable us to determine the duration of on-site FPC audits and to provide you with a costed proposal.
When you have completed the form it should be sent together with the supporting information (see MCS Application Checklist on p8) to:
mcs@kiwa.co.uk or by post to MCS Admin Team, Kiwa Ltd, Orchard Business Centre, Stoke Orchard, Cheltenham, GL52 7RZ.
The declaration at the end of the form may be signed either physically or digitally. Electronic submission of the form from the Applicant’s company email address also constitutes acceptance of the terms set out in this document.
At this stage of the process the organisation cited in Applicant Details below is described as the Applicant. If certification is completed successfully the organisation will be referred to as the License Holder.
The Applicant is the organisation that will be in contract with Kiwa Ltd and to whom the MCS Certificate will be granted. Please ensure that the name and address appear as you would like them on the certificate and in any publication of the product(s) certification status on the MCS website and any Kiwa Group website.
